CarePlus Creating Connections Group for TMS

The Care Plus NJ school-based services holds some scheduled psychoeducational services, such as an executive functioning group, which require permission from parents or guardians to participate.  The 7th and 8th grade groups will begin to meet on Monday 4/15/24 and the 6th grade group will begin on Tuesday 4/16/24.

THIS FORM must be completed and will serve as permission for the Tenafly Middle School student named to participate in the Care Plus psychoeducational program Creating Connections Group in the 2023-2024 school year.

The group will run weekly for 7 weeks. The 7th and 8th grade groups will meet on Mondays during their lunch and study hall class period (7th grade during period 6-7 and 8th grade during period 8-9). The 6th grade group will meet on Tuesdays after school from 3-3:45pm. The group will provide students a safe space to maximize social interaction opportunities that will build self-confidence through creating meaningful connections. In the group, students will work to gain self-monitoring skills, enhance appropriate responses to social situations, and relate to others through cooperative activities, games, and discussion with their peers.

The group will be facilitated by CPNJ Clinician Mackenzie Kuhl, LSW and will be held in-person at Tenafly Middle School. Students are expected to follow all school rules and regulations.

Space is limited. Please register by submitting this form no later than 3/28/2024. Submission of this form does not guarantee that your child will receive a spot in the group due to limited space.
