Health Requirements
MEDICAL REQUIREMENTS FOR SCHOOL ENTRY
Health history, a physical exam and current immunization records are required prior to school entry. Any student requiring a Mantoux tuberculin skin test (by the New Jersey Dept. of Health) must submit documentation prior to school entry. Please take this list with you when you visit your pediatrician and review it together.
IMMUNIZATION REQUIREMENTS
NURSERY AND PRE-K
► 4 doses of DTaP (age-appropriate)
► 3 doses of Polio (age-appropriate)
► 1 dose of MMR given on or after the first birthday
► A minimum of 1 dose of Pneumococcal given on or after the first birthday
► A minimum of 1 dose of HIB given on or after the first birthday
► 1 dose of Varicella vaccine given on or after 1st birthday
- Physician diagnosis of disease or lab evidence of immunity to chickenpox
► 1 dose of flu vaccine to be given by November 30th of each year
KINDERGARTEN AND GRADE 1
► 4 or 5 doses of DTaP -one dose given on or after 4th birthday
► 3 or 4 doses of Polio -one given on or after 4th birthday
► 2 doses of Measles, Mumps and Rubella given on or after pt birthday.
- The 2nd dose is recommended between 4 to 6 years of age; however the minimal interval after 1st dose is 1 month.
► 3 doses of Hepatitis B vaccine
► 1 dose of Varicella vaccine given on or after 1st birthday
- Physician diagnosis of disease or lab evidence of immunity to chickenpox
GRADE 7-Entering 9/1/26 or after
► 1 dose of Meningococcal vaccine given at 7th grade or higher
- Routinely recommended at age 11-12 ; Minimum age of 1O *
► 1 dose of Tdap vaccine given at 7th grade or higher
- Routinely recommended at age 11-12; Minimum age 10 *
HIGH SCHOOL
► All entrants must have immunization requirements listed above completed.
Also, note that according to NJ State Law, the age requirements are very important and are
strictly enforced.
