Overview: THS/TMS Sports Registration Process
Any THS or TMS student who wishes to participate in a fall, winter, or spring sport must follow the two-step registration process outlined below.
STEP 1: Visit the Genesis Parent Portal on www.TenaflySchools.org to register for your desired sport online, and to review/sign the necessary sports participation waivers.
STEP 2: Print, Complete, and Submit a hard-copy of either the PPE (Pre-participation Physical Examination) and/or the HHQ (Health History Questionnaire) and submit directly to either the THS Athletic Office or the TMS School Nurse (for middle school sports) by the announced submission deadline date.
Please be aware that the Athletic Office must have on file, or be provided with an update of, a valid sports physical in order to be able to medically clear each student-athlete to participate in a Tenafly school sport. Students will not be able to tryout or participate in a sport until their medical form is received, reviewed, and cleared.
For clarification regarding which medical form needs to be submitted (PPE or HHQ), the term "Valid" is defined as a medical exam that has been done within 364 days prior to the official season start date for the sport they wish to participate in.
- A PPE (4 page) medical form must be submitted by students who need to provide us with an updated physical exam (i.e. students whose physical has expired and is therefore no longer valid).
- An HHQ (1 page) medical form must be submitted by all students if they plan to participate in a sport.

No other HHQ forms can be accepted to clear/approve student athletes for sports participation.
